Showing success in assessment and improving safety culture on the DLR: Industrial application
We presented with David Nichols of KeolisAmey Docklands (KAD), a paper on “Showing success in assessment and improving safety culture on the DLR: Industrial application” at the the sixth international Human Factors rail conference in November 2017. KAD wished to assess the safety culture on the Docklands Light Railway (DLR) and commissioned Greenstreet Berman to both conduct the assessment, and to support the development of an improvement strategy and action plan. KAD have since been in action to improve safety performance and culture across the DLR.
‘Supervision+’ – scoring high marks for compliance through supervisor non-technical skills
In recognition of the role supervisors play, and in response to a desire to further improve safety performance within a licensee facility, Greenstreet Berman developed a tailored programme called ‘Supervision Plus’ (Supervision+), to help supervisors develop the required non-technical skills for successful performance and management of procedural compliance.
Developing a practical self assessment tool for measuring and managing safety culture
Developing an enhanced self assessment safety culture measurement tool and guidance on how to implement best practice solutions for continuous cultural improvement. This tool has been trialled by rail group members and found to be robust and credible.
Organisational drift – a challenge for enduring safety performance
This paper, which is based on recent work undertaken for the UK nuclear industry, considers whether organisational drift is a valid and useful concept, and also considers how it might be countered. The paper presents some observations concerning the types of defence that could detect early signs of organisational drift, and concludes that organisational drift is something that should be considered by all high-reliability organisations.
Preventing the propagation of error and misplaced reliance on faulty systems: A guide to human error dependency
Human error dependency is particularly serious because it can “defeat” multiple independent safety defences, thereby undermining safety in what would otherwise be a high reliability system. The guide draws on practical examples of error defences and accident case studies to show that it is practical to guard against these types of errors.
A guide to enhancing process safety and plant efficiency through the competence of Control Room Operators (CROs)
CRO skills are critical to avoiding errors that degrade plant performance and safety as well as handling faults and emergencies. Key CRO competencies include understanding plant state, gathering and interpreting data, process monitoring and fault detection and handling process upsets. CROs must understand process and plant function across normal, abnormal and emergency operations, detect process abnormalities, maintain situational awareness of plant status and take action to optimise plant operations across these conditions.
Human Factors good practice guide to managing alarms and alerts
When designed appropriately, audible and visual alarms and alerts can be very effective. However, if designed poorly, without consideration of the operator’s needs and abilities, or their relationship to other alarms and alerts within the environment, they can be overwhelming, confusing, and annoying. A review by Human Factors (HF) experts in the rail industry identified no specific guidelines for the design and fitting of alarms and alerts in the rail industry. Following stakeholder consultation, this project was carried out with a view to addressing that gap.
Understanding people’s attitudes towards fire risk
Research was carried out into the attitudes of vulnerable groups to fire risk. Interviews were carried out with community fire safety advocates and focus groups were conducted with a number of vulnerable groups. The research reported on the attitudes of these groups to fire risk, encouraging behaviour change and the role and impact of community fire safety advocates. The report includes recommendations for developing the work of advocates and community fire safety more generally.
